Intent to participate in 2026 - 2027 ESSA Programs.
Please use this form to indicate your school's intent to participate in the 2026-27 ESSA programs. For each ESSA program listed in this form, private school officials should indicate their intent to participate by selecting the most appropriate option for the 2026-27 school year.

Additionally, use this form to specify the percentage of funds, if any, you would like the District to withhold for Supplies and Materials.

Please complete one form per BEDS number. Forms that include multiple BEDS numbers will be rejected.
