Please use this form to indicate your school's intent to participate in the 2025- 26 ESSA programs. For each ESSA program listed in this form, private school officials should indicate their intent to participate by selecting the most appropriate option for the 2025-26 school year.

Please complete one form per BEDS number. Forms that include multiple BEDS numbers will be rejected.
